I have had no reason to use PDFtoMusic before, and now I am embarrassed as to not know how to use it.  
Firstly, I noticed that my version 2.1.0f (32-bit) was outdated, so executed a software update via the program.  It downloaded the 64-bit version.  
I deleted this and downloaded the 32-bit version: the demonstrations play OK
 
With the file shown below, nothing happens with Performance > Play .  
There are lyrics attached to the bottom of this PDF.  The documentation refers to a toolbar and mentions clicking on the Virtual Singer.  
 
Is a toolbar supposed to be visible here in this version?   (I see it can be customised via Setup)

What does Setup>Toolbar display ? There should be a "factory button". Now, perhaps installing the 64-bit version  left some wrong file in the disk. Pergaps looking in the Windows applications folder (and user's Myriad folder) and deleting all that concerns PDF2M beforre re-installing the 32-bit version could help. On my machine (Win XP 32) I do get the toolbar.

Well, I've just tried to open the file into PDFTM (standard and pro versions). I've got the toolbar, but the file doesn't play.
I exported it (.myr or .xml) but it doesn't play in HA. I removed the coda sign at the beginning and managed to play the score. However, I must say it is far from being "clean" (multiple free objects scattered all over the place).

Well-spotted; thank you for finding that hidden coda; I have reported that the PDF does not show this.
It is this coda that is responsible for the list of zero bars to be played.
 
The .myr conversion is indeed a mess (much coming from Title/Comments/Composer.)
And why is there a G7 grace note in bar #1 ?
 
Could you please show me what the toolbar looks like; is it masked (behind) in the screenshot above) ?

I have no grace note in bar #1 (neither in PDFTM nor HA). Now, for some reason I can't shoot a screen copy (probably because I'm running HA in  a Virtual Box) but I can say the toolbar has the same appearance as in previous versions of PDFTM.
Once the coda has been deleted the file plays in HA and if one deletes the Title/Composer/Reamrk fields  and the dozens of useless free objects, the score is "readable" (I attach the file)

I just tried to install p2m both standard and pro. In the standard version the toolbar is visible, but in the pro version it is not. I don’t want to pay the licence until I know how to make the (working) toolbar visible.  
 
So how can I make the toolbar visible?  
 
Is there some ini file that needs to be removed after several installs and uninstalls?

If you are running under Windows,
you can go to %APPDATA%\acampref\Myriad\PDFToMusic directory (Start > Run)
 
Make a copy of this directory, or simply rename it, and run PdfToMusic. It should run as a fresh new install.
